If a regulator, incident investigator, insurer, or public safety authority asked you today:

“Who is accountable for every AI-influenced operational decision across your critical systems — and how would you defend that accountability under scrutiny?”

Could you answer in a single, defensible sentence?

Not a framework.

A sentence that holds up under pressure.

Most organizations do not yet have a clear answer — and that gap is where oversight exposure becomes operational, legal, and safety risk.

Boards and audit committees in several sectors have already begun tracking this question in internal reviews.

Why This Gap Has Real Consequences

When AI-influenced outcomes inside critical infrastructure are questioned, the consequences are rarely abstract.

They show up as:
